Dear Patient,
Thank you for entrusting me with your medical care. It has been a true pleasure to work with you. My medical practice will close at the end of 2021. The last day that I will be seeing patients is December 29, 2021.
Please select a new physician as soon as possible. Several board certified physicians have agreed to continue to provide you the care that you need. Their names and contact information are listed on the back side of this letter. I am happy to forward a copy of your records to any physician that you choose after I receive your written authorization on the enclosed form. Personal record requests will be subject to state approved charges for copying and postage.
After my practice is closed, you may request a copy of your records by filling out this form HERE. However, please be advised that the website will be taken down on September 30, 2022. All correspondence and record release forms need to be by mail and sent to Keil Urogynecology P.O. Box 461292 Denver, CO 80246.
Again, it has been an honor and pleasure to care for you over the last 23 years. I have truly enjoyed the relationships that you have given me. Thank you for the gift.

Operative Reports can be obtained directly from the hospital that you had surgery at without any charge. This is the most important piece of your medical record. The majority of surgeries were at Rose Medical Center or Rose Surgery Center.
